Message From Our Dear Miss Gina ...

Dear 4N1,

Please note that you are supposed to receive a Travel Declaration Form from the school. Please fill up the form and submit it to me on 29th June.

If you have not received the form, please kindly email the school changkatchgi@moe.edu.sg. Please remember to include your name and class, and request the school to send you a copy of the form. This has to be done by Friday, 26th June.

Please inform me the moment that you feel unwell so that I can let the school know. If you have traveled to an affected country and is not feeling well, please call 993 for medical assistance. You should also stay at home until you are fully recovered. Please do not go out, e.g. shopping, for meals, take MRT or socialize with others until the doctor tells you that you are well.

Last but not least, please always remain contactable and make sure you have a thermometer with you when you come school.
